The Youth in Transition Program provides support for youth ages 14-25 who are in the process of transitioning out of the care of Children's Aid Society (CAS).

Services Overview

Help youth obtain skills to support them in their journey into the adult stages of life
Our Youth in Transition worker actively engages with youth from all our communities to deliver culturally appropriate programming and provide supportive services to youth transitioning out of care and into adulthood.

Create individualized transition plans
Our Youth in Transition Worker partners with each youth to develop a personalized transition plan created to their unique strengths, needs, and goals. These plans serve as roadmaps to adulthood, helping youth:

Set meaningful short-term and long-term goals

Identify supports and resources to achieve those goals

Plan for housing, education, employment, and wellness

Build connections with community services, and cultural supports

Access services to improve long-term outcomes
Connect youth to culturally relevant services and supports that strengthen long-term outcomes in housing, education, employment, and wellness. These connections empower youth to build stable, fulfilling lives rooted in their goals and community values.

Working collaboratively at all levels
Mnaasged Child and Family Services are committed to working collaboratively at all levels using a family-centered, strength-based, and child-focused approach, and supporting care providers in offering safe, nurturing environments for youth.
Mnaasged Child and Family Services continues to support Eelunaapeewi Lahkeewiit (Delaware Nation) in planning and developing youth services and supports through Post Majority Care funding.
